Annandale surgery is working together with Carers in Hertfordshire to offer support to patients of any age who have caring responsibilities. This includes parents of children with learning difficulties or young people who are carers to parents or other family members. Many people do not see themselves as carers. They are simply someone's parent, child or friend.
If you look after someone who is ill, frail, disabled, has mental health problems or misuses drugs or alcohol YOU ARE A CARER. Caring can be a 24/7 job that can be demanding and isolating. Carers in Hertfordshire will be able to connect you with the relevant organisations and support networks. They offer advice and you will also receive regular information sent directly to you.
Complete a carers form from reception, we will update your records and pass your information onto Carers in Hertfordshire. All registered carers can pre-book appointments for themselves and the person they look

Carer Coffee Mornings: Carers Coffee mornings are arranged by the 3 GP practices in Potters Bar, Annandale, Highview and Parkfield and the Friends of Parkfield Patient group.
They are an ideal opportunity for carers to get together over coffee for advice and support. They are free and are held at St Johns House, The Walk, Potters Bar 10.30 - 12 noon. No booking required, just drop in. You can bring the person you care for, or for respite assistance contact CROSSROADS on 0208 905 1158.

Crossroads Care: Crossroads Care is a national network of local charities. With over 5,000 trained professionals, they are Britain’s leading provider of support for Carers and the people they care for. They provide a flexible and adaptable service, offering a diverse range of services for all age groups, disabilities or health conditions.

Alzheimer's Society: Alzheimer’s Society is the UK’s leading care and research charity for people with Dementia and those who care for them. A Dementia Support Worker can visit you at home to offer emotional and practical support.

Family Lives: Family Lives is a free 24 hour confidential helpline offering support for complex and difficult family issues. They can help with all aspects of family life that include all stages of development including bullying at school, behaviour and mental health concerns of both parents and children.

MIND: Mind is a Mental Health Charity which can offer advice and support if you are living with someone who has a mental health problem. Having access to the right information about a condition, treatment options or practical issues is vital.

Hertsmere Community Transport Schemes
Hertsmere Community Transport offers Ring & Ride which is a door to door transport service for people in Hertsmere who are unable to use public transport. It is primarily for local trips to leisure facilities and operates on weekdays only. Bookings can be made between one and seven days in advance by ringing 020 8207 5115 the office is open 9.30am to 12.30pm Monday, Wednesday and Friday There is a small charge.
Shopper Scheme is also a door to door service where people are picked up from their homes by minibus and taken to the local supermarket. There is a small charge.

Hertsmere Community Transport has a Wheelchair Accessible Car for the use of Hertsmere residents who are wheelchair users. The car can accommodate up to 3 passengers. It is usually driven by a volunteer from the Community Car Scheme. However, the car may be driven by friends or relatives of wheelchair users. It is available some weekdays and at the weekends. There is a small hire charge and mileage rate. At least 3 working days notice is needed. Call 020 8207 5055 9.30 – 2.30 Monday to Friday

At Annandale Surgery we are here to support you as a Carer. Bernadette is our named Carers Champion.
You can pre book GP and Nurse Appointments for yourself and for those that you care for. Just let reception know that you are a carer when you book an appointment.
We appreciate that your health is important for you and the person that you care for and we offer all our carers a health check.
The Health Check consists of a few simple tests that will give you and your GP or health professional a clearer picture of your health in particular your risk of developing diabetes, heart disease, a stroke or kidney disease. All the tests are simple to carry out, and there will be time to discuss the results with your GP or health professional afterwards.

Young Carers are children and young people between 9-18 years who help care for family members who may be ill, have a physical or mental disability, or drug or alcohol problem.

Carers in Herts welcome carers who have lost the person for whom they cared.
They run a series of free sessions which we hope will help you to:
